At Carroll Family Medicine, we make every attempt to make your visit friendly and comfortable.

When you arrive you will be greeted by our warm and friendly staff. Our Physician and Physician's Assistant will spend time listening and learning about your unique personal and family medical history. We encourage questions and comments. You will share in planning your health care in a comfortable, supportive environment.

Our goals are to provide you and your family with:

Since our physician is Board Certified in Family Practice we can diagnose and treat most medical conditions for your family - from newborns to seniors.

**NEW INCLEMENT WEALTHER POLICY - STARTING JANUARY 26, 2015 - IF CARROLL COUNTY PUBLIC SCHOOLS ARE TWO HOURS LATE OR CLOSED FOR THE DAY, THE OFFICE WILL OPEN AT 10 AM.  FOR PATIENTS WITH APPOINTMENTS SCHEDULED BETWEEN 8:30 AND 9:45, YOU WILL BE CALLED BY A MEMBER OF OUR STAFF TO NOTIFY YOU OF THE CHANGE.**

Saturdays & Sundays,
New Year's Day, Good Friday, Memorial Day,
July 4th, Labor Day, Thanksgiving, and Christmas Day.




After Hours Emergencies

In case of an after hours emergency, please call  410-790-9549, the cellular telephone number for Dr. Uggowitzer.  If the Doctor does not answer, please leave your name, telephone number where you can be reached, and a brief description of the medical problem.  If you have not heard back from the provider in 15 minutes, please call again.  We will make every effort to return your phone call as soon as possible.  If there is a true emergency, do not hesitate to call 911.  For routine matters, like prescription refills or scheduling an appointment, please call the office after 8:30 on the next business day.

Appointment Scheduling

We try to accommodate the needs of our patients.  We will try and schedule your appointment at the most convenient time possible.  If you need to be seen the same day, we will make every effort to work you in with your medical provider. Otherwise, we will schedule you with the provider available. Please call  410-239-0406 to make an appointment.

~ A Notice to Our Medicare Patients ~  Within the first 12 months of your initial enrollment, Medicare will cover a "Welcome to Medicare Physical".  Medicare will provide for a comprehensive physical examination annually.  Please refer to your Medicare Handbook to see what other specific services may be covered.  At these visits, you will also be asked to sign a Medicare Waiver Form at the front desk.  This is called an ABN or Advanced Beneficiery Notice.  It states that you are aware that the services you are receiving are not covered under Medicare, and that you will be responsible for the bill.
